Item must be unworn with no odors, smoke, perfume, etc.) and all original tags must be attached.
Item must be returned postmarked within 30 days of receiving item.
Item must be purchased from our DHStyles.com web store (This form is NOT for eBay purchases)
Return the item(s) together with this completed form and a COPY of your order sales receipt/invoice
NOT paid with a RMA store credit. Previous RMA credit used WILL become void (forfeited) with this return.
